Waiver Program Info

The Abilities Unlimited Waiver Program is a component of the Medicaid ACS (Alternative Community Services) Home and Community Based Waiver Services (HCBS).  This state and federally funded program is for individuals with intellectual and developmental disabilities with an age of onset before the age of 22.  It is an alternative to ICF/DD, nursing home placement or institutional care (restrictive settings).  The Waiver Program provides individuals the opportunity to live as independently as possible in the community of their choosing.
 

Abilities Unlimited of Fort Smith, Inc. specializes in Supported Living Services component of the ACS waiver program.  Providing direct care professionals to work in the individuals home and community on goals and objectives desired by the individual that are identified in the PCSP (Person Center Service Plan).  Supported Living helps individuals to develop and keeps and/or improve self-help, socialization and adaptive skill necessary to live successfully in their home and community with their family or in an alternate living residence or setting.

Community Involvement

Abilities Unlimited staff provides services and supports to encourage the client's involvement in their local community.  These services include but are not limited to, recreational activities reflecting personal interest and socialization opportunities with other clients.

Transportation

Non-medical transportation is provided to enable individual to gain access to their community, activities and other resources as part of the PCSP.  We assist in locating appropriate transportation options for medical appointments.

Section 8 Rental Assistance(HUD)

Assistance is provided to the client in obtaining affordable rental support.  Our program will assist with locating and coordinating the services for qualifying individuals.

Supplemental Nutrition Assistance Program (SNAP)

SNAP offers nutrition assistance to eligible, low-income individuals.  SNAP provides a monthly supplement for purchasing food.  Our program will assist with applying for services and coordinating scheduled meetings with a SNAP representative.
